Yes. Evolve Health was formerly Modern Foot & Ankle. We were not acquired; the new name reflects our growth into multi-specialty care. The same doctors, offices and leadership continue to serve patients across Florida and Texas.
Podiatry: heel pain, plantar fasciitis, bunions, hammertoes, flat feet, Achilles tendinitis, diabetic foot ulcers, sports injuries and arthritic foot conditions. Physical therapy: recovery after surgery, balance disorders and musculoskeletal conditions. Nephrology: chronic kidney disease, high blood pressure and diabetic kidney disease. Rheumatology and an infusion center are next.
Yes. Our podiatric surgeons perform minimally invasive and reconstructive foot and ankle procedures, including bunion correction, hammertoe repair, Achilles tendon repair, neuroma removal and diabetic limb salvage. Every surgical plan starts with a thorough evaluation to see whether conservative care can resolve the problem, and is followed by a recovery plan supported by physical therapy.
Yes. Licensed physical therapists at select Florida offices provide rehabilitation after foot and ankle injury or surgery, and care for balance, mobility and chronic pain. Your therapist works from the same plan as your Evolve Health podiatrist or surgeon.
Most specialty care is fragmented: separate practices, separate paperwork, separate plans. Rheumatology, for arthritis, gout and other inflammatory conditions, and an infusion center in Florida for in-office infusion therapies. An ambulatory surgery center in Florida is also planned. Each is designed to be reached from your existing Evolve Health care, not through an outside referral.

Evolve Health accepts many major insurance plans in Florida and Texas, including Aetna, Cigna, Blue Cross Blue Shield and Humana. See the insurance page for the full list, or contact us if you do not see your plan. Our billing team verifies coverage and explains any copays before your visit, and self-pay rates are available for patients without insurance.
