Evolve Health began with a small group of physicians and podiatrists committed to delivering high-quality, accessible care. What started as a single podiatry practice grew into a leading podiatry group, and has since expanded into a connected, multi-specialty medical group serving communities across Florida and Texas.
Our goal has stayed the same through every stage of that growth: care that is easy to access, supported by modern technology, and delivered by providers who put clear communication and the patient’s experience first.
As our services and locations continue to grow, we stay focused on coordinated care that helps patients feel confident in their treatment and supported at every step.

About Evolve Health and Modern Foot & Ankle.
Yes. Modern Foot & Ankle became Evolve Health as the group expanded beyond podiatry. Your doctors, offices and phone numbers are the same; the name reflects the addition of physical therapy, nephrology and more specialties to come.
Across Florida and Texas, with offices in the Orlando, Tampa Bay, Sarasota and Palm Beach areas of Florida and the Houston area of Texas. The locations page lists every office with its providers, specialties and hours.
Clinical decisions are made by the physicians who treat patients, not by a management company. Our leadership team includes practicing clinicians, and our care philosophy is written by them.
Podiatry, physical therapy and nephrology today, with more specialties being added as the group grows. Because they are part of one group, your providers can coordinate your care directly.
We accept many major plans in Florida and Texas, including Medicare and many Medicare Advantage plans, and we verify benefits before your visit. Search your plan on the insurance page or call us.
